Poem - THE CALLOUS SOUL The callous soul - RGK She wore down till her last shade ,             when you complained of the stain . Through the shattered mirror,          she gazing over her wilted weather's pain . Streams from her eyes trace down her parched lips ;   they fumed as if she tasted the ocean in her dips. Because all I wanted was Doremon’s magic door at that point. 25 July 2023, I couldn’t take it anymore, the heartache was unimaginable. I was no longer able to move a chair on my own or remove my own clothes, so insisted Shreya to cut them and rip them off . I gave her nightmares (“I’m sorry shrey I made you to go through this”).       AM I JUST A KID?  Everyday there was this wave of thoughts reminding me to what lengths I was taking myself in desperation. It made me question myself why is it so important for me to look perfect? Why am reaching out to these many lengths just for lips? Is it really just for the appearance or is it an unexplainable fear to accept the fact that I’m falling sick?   The grave ignorance! People often strive to present themselves as flawless and perfect, with society valuing clear, bright skin as a symbol of wealth and cleanliness, while associating darker shades with ignorance. However, the reality remains unchanged; an 11-year-old child should not feel pressured to have a flawless complexion or feel guilty for engaging in sports under the sun. Scars and pigmentation do not hinder one's ability to achieve success or enjoy memorable moments with friends, unaffected by body odor or skin imperfections. ( My experience during these incidents took place while I was away from home (ie.India) and studying medicine in Georgia.)   The symptoms initially manifested in my mouth and throat . I distinctly recall waking up one morning on March 25, 2023 , with a dryness in my throat. I attempted to ease the discomfort by drinking water, but swallowing became incredibly difficult , leading me to believe it was just a common case of dryness after a long break. However, as the days went by, the sore throat intensified , and I became increasingly aware of the presence of recurring mouth ulcers , which is referred to as blisters. These blisters caused me great distress, as they multiplied in both number and size, inflicting constant pain. What is Pemphigus vulgaris and is it common? : PV is an uncommon , potentially life threatening autoimmune disorder characterised by extensive erosions on healthy skin.   2. How do you diagnose  Pemphigus? : Skin biopsy is done from the fresh blister(fluid containing bubble) , microscopy of perilesional skin.   3.What are the follow blood test to keep in track? : They mostly need frequent monitoring of the CBC (compete blood count), LFT (Liver functional test) and Dsg1 and 3.   4.What is dsg1 and 3 ? : Evaluation of desmoglein 1 and 3 ie dsg1 and 3 auto-antibodies seem to be more useful for monitoring the extend of the disease and severity by using enzyme-linked immunosorbent assay ( ELISA ).   5. Tips and tricks on managing Pemphigus vulgaris

  Tips and tricks that help me going!  Biggest alteration in my daily life after Pemphigus vulgaris that affected more were the basic necessities. The items added down are ones recommended by my doctor and some products that are personally helping me till date.  How do I protect my skin from friction during open wound? :  While sitting on the chair and while sleeping, I use under pad sheets (only change is  use the plastic side of the pad ). Yes, they produce heat but they are best for frictionless and better than cloth rubbing on the wound. It is impossible to carry ourselves without FRICTION.       What clothes can be worn? :  I’ve been using hospital gowns at home, front opening as they will not affect my under arm (axila) and bigger the size bigger the comfort as they don’t disturb with the movements. For hospital visits cotton dresses preferably front opening and bigger in size than normal.             ...

  Define:  A rare autoimmune disease that is characterised by painful blisters and erosions on the skin, long term condition that causes the immune system to fight against the body’s own cells in the same way that it fights off invading germs .   Uncommon symptoms: PV is more than just painful blisters and erosions; it is more than their assault on healthy cells and their capacity to absorb protein that is found between the patient's skin cells.  Undergoing a significant life-changing event impacts not only the physical aspect but also the mental, emotional, and social aspects. Despite consuming a high amount of protein in the diet, there is still a deficiency in protein levels, necessitating the administration of albumin and plasma protein transfusions.
